Important Notice Regarding Affordable Care Act (ACA) 1095-B and 1095-C Forms
Per the Paperwork Burden Reduction Act, the Roman Catholic Archdiocese of Boston Health Plan will not be mailing the 1095-B and 1095-C forms to eligible participants for 2025. Instead, copies of these forms will be mailed upon request. Forms will also be available in BAS/MyEnroll by March 31. Instructions on how to access these forms are available here. Please contact the Archdiocese of Boston Benefits Department by phone 617-746-5640, e-mail [email protected], or mail 66 Brooks Drive, Braintree, MA 02184, to submit a request. Blue Cross Blue Shield of Massachusetts will provide 1099-HC forms to eligible participants by mail. Copies can be requested by phone at (800) 832-3871 or retrieved in MyBlue after January 31, 2026.
